TOURIST VISA Requirements:
Applicants must submit an ITINERARY FORM and the following documents are submitted in two (2) sets – one original and one copy
1. Passport or travel document accepted by the Schengen countries valid for a period of 3 months beyond the applicant's last day of stay abroad. The passport must have a blank page on which the visa will be affixed.
2. Proof of permanent residence in the United States: U.S. Alien Registration Card or valid long-term U.S.
3. One recent passport size photograph (2x2 inches in size, full face, front view, color).
4. One application form completely and clearly filled out, to be signed by the applicant in the presence of a Consular Officer.
5. Proof of sufficient funds to cover the expenses of your planned stay (most recent bank statement/s)
6. Employment verification:
a) Letter from employer, or letterhead, stating position, salary and that the vacation/leave has been approved. The letter must be signed and dated
b) If you are self-employed, present the valid business license, certificate of incorporation, and latest tax return concerning business.
7. Flight reservation from travel agent. The round trip ticket, plus one copy, must be presented when picking up or mailing out the passport. If it applies, present eurail tickets, car rental reservations, internal flight reservations. (Please request from FKC)
8. Lodging:
If you are traveling with an organized tour, present a letter from your travel agent stating that you are a member of the group, and a copy of the tour itinerary (Please request from FKC)
9. Proof of purchase of Worldwide Travel Insurance. (Please request from FKC)
10. Handling fee will be paid in exact amount in cash or money order upon presentation of the visa application regardless of the outcome of the request. (Exchange rate will be adjusted every three months.)
11. Please provide a self-addressed (from yourself–to yourself) pre-paid/stamped envelope (Express Mail, FedEx), if you wish to have your passport mailed back to you.

The fee must be paid at the time of the application, and is non-refundable REGARDLESS if the VISA will be issued or not.
